Pastoral
This reference form is to be completed by the applicant's pastor, elder, or deacon who has known the applicant for at least one year and is not a member of his/her immediate family. Fill out the box at the top of the reference form before giving it to your reference. As a courtesy, please provide the individual with an addressed envelope and postage. Reenrolling students must submit a pastoral reference if two or more semesters at Calvary have been missed.
